Below you will find various forms used by Global Insights, along with a description
of the purpose of the form. In order to print out these documents, you will
need to use Adobe’s Acrobat Reader software. Get this software free by
clicking on the Get Adobe Reader box. Then come back to this page. If you
already have the Acrobat Reader installed on your computer, you can open the
forms now.
Global Insights Student Application –
This form is used by students who wish to apply for the Global Insights High
School Exchange Program. The application is 19 pages and includes the
complete terms and conditions and rules of the program.
Student Travel Notification
Form - This form must be completed to document student travel 1) with the
host family or Area Representative during the program in which school
absences will NOT be required, and 2) with natural parents and/or designated
adults over 25 at the end of the program only. This form must be submitted to
the local Area Representative prior to commencement of travel.
Student Travel Request Form
- This form is to be completed if a student would like to request permission
to travel without his/her host family or Area Representative, or in cases
where the student requests to miss school to travel.
Natural Family Visit
Request Form - This form must be completed for Global Insights to
consider granting program approval for natural parent visits to the student’s
local community PRIOR to the student’s release from the program.
Program Release Form - The form is
used to release/dismiss a student from the Academic Year Program (AYP) of
Global Insights.
