Application Forms

To officially apply to become a Global Insights Area Representative, you must complete the Area Representative Application, provide 3 references, and submit to a Criminal Background Check. In order to download the following forms, complete them online, and save the data to your computer, you must have Adobe Reader Version 8 or higher installed on your computer. (If you do not have Adobe Reader Version 8 or higher, obtain/update this free software by clicking on the “Get Adobe Reader” box below. Then come back to this page.)

IF YOU HAVE TROUBLE OPENING THE FORMS; YOU MAY NEED TO UPGRADE THE ADOBE READER.  Downloading the READER 9 is FREE and easy.  If you have version 8 or higher, you can complete the forms online, save them to your computer if you don't have time to do it all at one sitting, and email the completed forms.  DOWNLOAD ADOBE 9 READER NOW
 

If you already have the preferred version of Reader installed on your computer, you can open the forms below by clicking on the related link. Users can use their computer to type their responses directly onto the application or can print out a blank form and complete it by hand. Whether completing the forms online or by hand, forms should be signed and dated and the originals mailed directly to Global Insights:

Download the Area Representative Application
Complete the Area Representative Application on your computer or print out a blank copy and complete it by hand. If you complete the application on your computer and have Adobe Reader Version 8 or higher, you can save the form to your computer’s hard drive.

Download the Character Reference Form
Open the Character Reference Form and enter your name as the Applicant. Next, either print or email a copy to each of the three people you wish to ask to provide a reference on your behalf (neighbor, co-worker, pastor, school official, etc. but not a relative).

Download the Criminal Background Check Form
In order to provide the information necessary to run a background check, you can either use the secure Criminal Background Check Portal, or you can download the Criminal Background Check Form and complete it, either on your computer or by hand, sign and date the form, and mail it to Global Insights.

Download a W-9 Tax Form
Area Representatives are classified as independent contractors, not employees of Global Insights. Earnings of over $600 per year will be reported to the IRS using a Form 1099. Use the W-9 Tax Form to authorize us to use your social security number for this purpose.
